If you have United Concordia Elite, our team will verify your benefits and submit claims directly to help you maximize your coverage. Patients with other insurance plans are welcome to visit us out of network. We’ll provide a detailed treatment estimate and all the necessary documentation for you to submit a claim to your provider for possible reimbursement.
